The NYS Transition Partners develop family friendly informational resources to enable people with disabilities and their parents to better understand programs, services, needs and promote independence and self-advocacy.
The NYS Transition Partners conduct two statewide conferences and provide scholarships for parents and young people to attend.
Online/phone opportunities are provided twice a year for the community to learn information that will promote independence and self-advocacy for youth with disabilities
A monthly digest of news, perspective, resources and events for New York State families, youth and transition professionals is distributed via email across the state.
A diverse group of experts from various organizations across New York State who share their technical and professional expertise to identify needs, priorities, and opportunities

The NYS Transition Partners develop family friendly informational videos to enable people with disabilities and their parents to better understand programs, services, needs and promote independence and self-advocacy.
The NYS Transition Partners shares success stories about preparing for or finding a job, navigating postsecondary education, or identifying a community resource or piece of assistive technology.
NYS Transition Partners is a six-year collaborative project funded by the Rehabilitative Services Administration to help New York State young adults with disabilities and their families navigate the transition to productive, independent, and meaningful adult lives. As part of this project, the Transition Partners plan on funding four local initiatives for $2,500 each throughout New York State for projects taking place between January 1, 2020 and September 30, 2020.
To help young adults with disabilities and their families, especially in underserved communities, to access and navigate vocational rehabilitation and other public systems that can help with financial stability, meaningful employment, and postsecondary education in New York State.
2020 AWARD WINNERS
Center for All Abilities (CAA)
Through the Blue Hat Baking Program, Center for All Abilities seeks to empower youth and young adults with all abilities to improve: baking techniques; daily living skills (independence, money management); vocational skills (time-management, safety guard); self-confidence; communication skills; conflict-resolution skills; and job-readiness. This funding will be used to fund supplies involved in cooking baking; provide training opportunities for program leaders; connect with professionals in the food industry or related field for consultation/support. This would contribute to greater opportunities for program participants to develop the skills they need to increase job readiness, obtain meaningful employment, and contribute to society.
The Center for Self Advocacy
This grant will fund a luncheon and community conversation on the challenges and opportunities facing youth with disabilities and their families who are making, or preparing to make, the transition from school to adulthood. The event is intended for youth across the disability spectrum and their families. The aim of this event is to foster cross-disability community, support, and empowerment through increased knowledge and understanding, not only about the services and supports available to young people with disabilities and their families, but also, about the history of the disability rights and self-advocacy movements.
Community Services for Every1
Under the proposed project, Community Services is seeking to develop an outreach resource that will help introduce transition age youth and their families to the experiences and opportunities available in the world of hospitality. In particular, we expect to create Be My Guest, an online video that will document how hospitality careers can represent vital pathways towards independence, inclusion and self-actualization for young adults of diverse abilities. While the video will be suitable for a broad audience of transition age youth and families, Be My Guest will be targeted towards youth and parents (a) who are only starting to consider their post-secondary prospects and/or (b) who are largely unaware of the rich continuum of vocational supports that could assist in discovering and achieving post-secondary goals.
University Settlement Society of New York
EXCEL engages individuals with special needs and their families through activities that promote
leadership, independence, healthy lifestyles, and creativity. EXCEL offers a range of closed-group courses focused on promoting physical health and coordination, life and social skills, and professional communication needed for the modern workforce. The students’ current project for which we are using this funding focuses on using Google and its form templates to support communication development and to assist students in their exploration of interests for future employment/volunteering opportunities. Students create personal gmail.com accounts for professional purposes while learning how to develop work place email etiquette. Students also use shared Google Docs to write collaborative short stories with their peers; Google Slides to create presentations they can share with a public audience; and Google Sheets to practice managing their finances. By the end of the class, each of the fifteen students enrolled in EXCEL’s Young Adult Technology and Communication will create a work portfolio that they can share with future employers and present to friends and family.
